for me it's Shadowman (but I guess some of you already knew that )
I also nominate Nightmare Creatures II, though it looks like shit it is actually quite good

i have yet to play nightmare creatures but heard it was like a beat em up which i like and i havent beat shadowman yet but the game sure has made me jump

Easily Illbleed. I never thought I could fire up a game I have never played before on an older (and awesome) system and actually GENUINELY get creeped out and nervous playing it at some parts. Then again it was my first time but still, it stuck in my mind.

illbleed is just amazing but u need to play both it and blue stinger
