there certainly are guides to making your own

maybe my elongated explaination here might help more people than just you, but:

the thing is, when you make your own, you're underestimating how thin and fragile the wire you're inducing is. its very, very thin. unless you're confident in your engineering abilities, sooner or later you will tug on the phone cable by accident and A.) disconnect the solder from the phone cable resulting in a NO CARRIER signal from the Dreamcast and therefore B.) have an anomalous electrical situation within your setup. which is very bad if you don't notice your homemade cable is broken.

i'm not endorsing the ebay sellers tho, PCwzrd13 makes his modems and cables with love, me personally i'd wait for his stock (idk what region you are)

Putting a step-up board inside a modem is not rocket science. You need to find a small enough board and follow the standard LVI schematic. This board is small and has multiple output voltage options https://www.aliexpress.us/item/3256801446546384.html
