If you have a bin/cue I'm not sure what you did. CDIrip usually outputs either two ISOs for each track, or two BINs for each track
You run isofix.exe using the CDIrip LBA value output for the larger track. For example, if the larger track is Track02, then it would output something like "LBA: 11702" before the window closes. You would then run isofix.exe on track02.iso or track02.bin, using LBA 11702
You would then open fixed.iso with ISOBuster, and extract the files
As for how to recomplile, you cd into the directory that has mkisofs and type
mkisofs.exe -C 0,11702 -V N00B_CDI -l -o data.iso c:\path\to\file.iso
You then would take data.iso, run isofix.exe again, enter data.iso and LBA 11702, then point it to bin2boot.exe and type fixed.iso
I'm not sure why you're using ultraiso, using programs outside of this guide is uncharted territory and I can't help you much
