Getting Dreamcast HKT-01 to Boot

Hello, I have finally been able to buy a Dreamcast HKT-01 debugger and when I turn it on I only get a black screen. I followed the SDK getting started guide but still cant get a picture. Does anyone know what the correct settings are on the device to use it as a console and play games without connecting a PC?

For reference, I have it setup as follows:

Slide Switch to the left
Rotary switch set to 4 for North America
DIP switch 1 & 2 are ON and 3 & 4 are OFF
For video I have a VGA monitor plugged in.

The settings seem about right. Do you get any audio? Are you able to connect it to a computer to confirm that you can see the drive and run a program on the unit? Do all three of the lights on the front flicker at startup for a while, going through the boot sequence, before settling down to just the centre LED?
